We wanted a "Consent to Travel" just in case someone at immigration in the Philippines or in the USA should ask for a consent form signed by my Filipino wife. It states that she is giving her permission for our daughter to travel out of the country with me.A couple of years ago a friend of mine recommended an attorney who I had visited once before. We called ahead to be sure he would be in his office in the afternoon. He said that he would be there. After visiting SM Clark City Mall, we took a jeepney to his office. His office is about 20 meters or about 60 feet from the Garden Dine Restaurant on MacArthur Highway.

He prepared the "Consent to Travel" letter for us and notarized it after my wife signed it. We made 3 originals just in case any got lost. We kept each in a separate place for safekeeping. As far as I know there are almost no safe deposit boxes available in the Philippines. I have never been offered one by any bank. I have heard that if one has a large enough balance in an account, one may be able to get a safe deposit box at a bank. Talk to bankers yourself and inquire about it. If you want a safe deposit box, maybe you can find a bank which will provide one.
Another attorney I visited was Willie B. Rivera. My first half hour consultation was free. Needless to say, that impressed me. He was, in fact, also a City Councilor (and may still be).
